Men’s sexual health………?

I am 57 years younger. Due to my 12 years ancient diabetic condition, plus ED and PE, my like life is at the end of the tunnel. I regularly see my urologist and he says with age, like life or sex life whatever you sum up, dwindles downward. He first gave me some papervine injections for the penis which I did not like it and did not used it much. Afterword I had a test done at Sunnybrook called ultrasound of the penis. It showed that an artery of the penis is blocking the veins of the penis. But at this time I am not concerned with this. Right now my concern is when I masturbate and when my semen comes out, right at the precise time, I get an agonizing, sharp needle like pain in the back upper part of my thigh., and that pain remians for the duration of exiting the semen and after word the pain is gone. But when I have this pain, it takes away the pleasure I should have when exiting the semen. Does any one know what it could be or has someone experienced it. Please help. Thanks.
